ELIGIBILITY FOR HOSPICE CARE
By meeting the following criteria, Will determine the eligibility requirements for a patient.

Based on the patient’s primary care physician and hospice medical director’s clinical judgment, the patient has been certified to have a limited life expectancy.
-The patient’s GP authorizes hospice care.
-The focus is now on physical, emotional, and spiritual comfort instead of healing measures.
